Yelp ratings of medical facilities may reveal local health inequalities

Counties with essential health care facilities that had lower ratings on Yelp also had higher death rates, the data showed.

“Patient experience and satisfaction with healthcare are increasingly recognized as important measures of the quality of healthcare, but data on these factors is less widely collected.” Daniel Stokes, MD, a researcher from the Penn Medicine Center for Digital Health and an internal medicine resident at UCLA Health, and colleagues wrote. “Patient experience could also be associated with county-level mortality, but these associations have not yet been well explored.”

Researchers conducted a retrospective, population-based, cross-sectional study of Yelp performance reviews for 95,120 essential health care facilities in 1,301 US counties from January 1, 2015 to December 31, 2019. Essential health care facilities were defined as those that offered services that Marketplace health insurance plans must cover: outpatient services to patients; emergency services; hospitalization; pregnancy, motherhood and newborn care; mental health and substance use disorder services, including behavioral health treatment; prescription drugs; rehabilitation and rehabilitation services and devices; laboratory services; chronic disease prevention and wellness services and management; and oral and eye care for children.

The conclusions, published in JAMA network open, indicated that at the county level, a one-point increase in an average Yelp score was linked to an age-adjusted average decrease of 18.05 deaths per 100,000 people (P <.001 the county-level average rating of facilities analyzed over study period one being worst and five best was stars.>

The researchers also noted that words related to time, such as “minutes”, “hours”, “finally”, “pending” and “schedules”; words to describe the payment, including “money”, “insurance” and “to pay”; and words related to interactions with other people, such as “rude,” “say,” or “say” appeared more frequently in one-star journals in high-mortality counties. On the contrary, words such as “friendly,” “kind” and “staff” were used more frequently in five-star reviews in counties with high mortality.

In a press release, Stokes said the findings show that “how patients perceive the care they receive has an impact on how they engage with health care and, therefore, on their health. own health and well-being ”.
